What Do I Need To Apply
To apply for Wisconsin Emergency Rental Assistance, you must complete the forms below and return them, along with copies of income statements, to Couleecap. The forms can be completed electronically and to sign you may type in your name and date.
Step 1: Complete an income attestation form. When completing this form, you are stating the economic
impact that the COVID-19 crisis has placed upon your household. Complete the income attestation form AND Landlord Consent for the Release of Confidential Information form.

Step 2: Provide proof of any income made within the prior 1 month. For example, if you are applying in March, you will need to provide proof of income for February (including payments received in March but for a period in February). Proof of income could be in the form of check stubs, Social Security and Social Security Disability, unemployment, child support print outs, Supplemental Security Income, Veterans Benefits, or Self-Generated Income and any other form of income.
